HCA Healthcare’s Mission Hospital had a long-operating outpatient pharmacy that was running a delivery program for its discharged patients. However, with no specialized meds-to-beds software, the pharmacy staff was limited to manual processes—whiteboards, spreadsheets and EHR note fields—to manage and track patients. These resulted in inefficient processes and ineffective results, with the pharmacy only able to reach a fraction of its intended patients.

Hospital leadership brought PharmaPoint on board and implemented XchangePoint’s MedAction module for meds-to-beds workflow with the goals of increasing pharmacy staff productivity and reaching a significantly higher percentage of the hospital’s discharged patients. XchangePoint’s integration into Mission Hospital’s Cerner EHR gave the pharmacy team the ability to prioritize and target groups of patients as the hospital determined its goals.

As XchangePoint replaced the prior ineffective manual processes, results were immediately evident through increased productivity. Since implementation, prescriptions delivered bedside have increased by approximately 2,200 dispenses per month, representing a more than 300% increase per meds-to-beds technician utilized.

In addition, XchangePoint reports show that pharmacy interventions also had a direct impact on patient outcomes. Patients experiencing interaction with a member of the pharmacy team have significantly reduced 30-day readmissions as compared to those in the control group.
