The ongoing phased partnership targets Next Level PRIME members, as well as spouses and dependents that have been diagnosed with diabetes, hypertension or hypercholesterolemia. XchangePoint’s MedOutcomes platform proactively helps identify patients that are nonadherent to prescribed treatment plans and tracks all interventions made by the care team.

PRIME members also have access to the Next Level weight loss program. PRIME in person services are available seven days a week from 9 a.m. to 9 p.m., but virtually (including telemedicine and self-service features through a mobile application) 24 hours per day, seven days a week.
